Select Baseball Registration Form - Form used to register Select Baseball Teams to be turned in at CFSA Office
Medical Release Form - To be completed and given to coach. Coaches maintain this on-hand for each player.
Coaches Application - To apply to coach in CFSA, each coaching candidate will need to go to the Registration system and register as a coach under your Family Profile.
